Arizona: check the right record
Medical doctors
Arizona Medical Board: Doctor Search
For a medical doctor, called an MD, Arizona’s board search shows licence details and links to the board’s written decisions about the doctor.
Check that the name and licence match your doctor. Read any linked decision for what the board said about that doctor.
Doctors with DO after their name
Arizona DO board: Find your DO
DOs are doctors whose licences Arizona checks through a separate board from the one for MDs.
Look for DO after the doctor’s name, then use this board’s search.
Nurses who prescribe
Arizona Nursing Board: check a licence
The Nursing Board links to checks for a nurse’s licence and certificates showing permission to provide care beyond a basic nursing role.
In the October 3, 2026 state rules, a nurse practitioner needs board permission to prescribe within their training.
A nurse practitioner is a nurse with more training. Ask for the nurse’s licence and the certificate that covers this care. Ask whether the board has given permission to prescribe.
Pharmacy permission
US drug agency: state pharmacy records
The Food and Drug Administration (FDA) points readers to state pharmacy licence records.
A pharmacy shipping medicine here from elsewhere must have Arizona’s pharmacy permit and a current permit or equivalent permission where it is based, under rules checked October 3, 2026.
Holding a pharmacy permit is no proof that all of its custom medicines comply with the law.
Ask for the pharmacy’s full name and permit details for both states. Use the state records linked here to check them.
Arizona law limits which past board decisions about a licence, including discipline for breaking rules, appear on board websites, as checked October 6, 2026.
A blank web record gives no assurance that the past holds no complaints or board decisions. More public records can be requested.
The rules for a phone or video visit
An exam from a distance
Arizona law: rules for remote care
Arizona can allow a prescription after a phone or video visit, but the person prescribing still needs a suitable exam and permission to prescribe that medicine, under rules checked October 3, 2026.
Ask the person prescribing how they will examine you and whether the rules for your medicine allow a remote visit.
Permission from another state
Arizona law: care from another state
A person licensed to give medical care in another state may apply to the matching Arizona board for permission to give phone or video care here, under rules checked October 3, 2026.
This permission requires a licence with no practice restrictions and Arizona’s checks on past penalties, insurance and other registration details.
That permission does not allow in-person care without an Arizona licence.
Ask the person treating you for the licence or registration used for your visit. Check it with the board for their profession.
Products: approval, compounding and labels
What compounding means
FDA: compounding questions and answers
Compounding is making or mixing a medicine to order for a patient.
A product made to order may fill a medical need, yet it lacks FDA approval, under information checked September 26, 2026.
Find out the reason for offering you a custom medicine. Ask how it differs from an approved product.
What an FDA badge means
FDA: understanding claims of approval
An “FDA registered” badge claims a business is on the agency’s register. A medicine can also appear on the FDA’s product list. Neither proves approval, under rules checked September 26, 2026.
Ask whether the exact medicine has been approved.
Problems with weight-drug copies
FDA: concerns about weight-drug products
This FDA page describes fake pharmacy names on labels, errors in measuring medicine amounts, and deliveries that were not kept cold enough.
Take the label to the pharmacist. Ask how to store the medicine and clear up any confusing directions.
Coverage and price terms
Medicare help with weight-medicine costs
Medicare: weight-drug coverage
The Medicare GLP-1 Bridge is temporary help with certain weight-medicine costs for qualifying adults 18 or older; it has weight, health and insurance rules and is separate from ordinary Part D drug coverage, as checked September 26, 2026.
Read the Bridge costs and eligibility notes
Use Medicare’s checklist with the person prescribing to check your eligibility. Ask your plan whether ordinary Part D or the Bridge fits your case.
A cash offer’s small print
Federal price page: semaglutide (Ozempic)
The starting price checked September 26, 2026 covers only the first two monthly supplies for new patients who meet the offer’s terms, and only for certain amounts of medicine per dose.
Ask whether your prescription qualifies, what the later refill price is, and which care fees are extra.
If a complaint is the next step
A complaint about an MD
Arizona Medical Board: complaint forms
The MD board’s complaint forms were checked on October 3, 2026.
Keep the doctor’s name and the papers related to your concern.
A complaint about a DO
Arizona DO board: filing a complaint
The DO board’s complaint page lets you file online or download a form, in information checked October 3, 2026.
Filing a complaint does not promise money or a set result.
